Shannon O'Dell, Curator of the Sindecuse Museum of Dentistry, has given me permission to post the link to the American Academy of the History of Dentistry's survey. I'm sure this will be a great resource for anyone searching for dental collections once it is complete. Her post on the RCAAM Listserv read:

If you recall, I sent out a message about 2 months ago indicating that the American Academy of the History of Dentistry is conducting a survey of all collections (historical, art, archives, libraries--all countries) containing materials related to dentistry. I got several people interested in being on the mailing list, however, I am sure there are more out there I didn't hear from. We want to get EVERY COLLECTION to report on any dental items (even small quantities) so we are encouraging you to participate. The survey should only take you about 15 minutes to complete and you can submit it directly from the website, or download the pdf and complete it. You don't even have to know what kind of dental instrument, etc. the items are since the form has category check boxes to make your data entry a snap.
I hope to see some of you in Denver. On the heels of that meeting, I go immediately to the annual meeting for the AAHD in Boston so if I learn from our survey coordinator that many of you have responded that would be wonderful.
Here is the link to the website to fill out the survey electronically.
